Shrinkage

Shrinkage is the loss of size a fabric goes through on first washing, as the fibres return to their relaxed state.

A pattern can be as accurate as you like and the garment will still come out too small if the fabric shrinks afterwards. Pre-shrinking before cutting is the only reliable answer.
